HealtheConnections Receives SHIN-NY Funding and Population Health Improvement Program Award


SHIN-NY Funding

On November 19, HealtheConnections began receiving funds from the Statewide Health Information Network of New York (SHIN-NY) to further its efforts in expanding the reach of the health information exchange (HIE) and improving healthcare in Central and Northern New York. HealtheConnections is a Qualified Entity (QE) of the SHIN-NY, designated to link multiple providers in a region via the statewide HIE and provide value-added services to those participants. 

In March 2014, the New York State Legislature voted to support the SHIN-NY in building a public network for doctors and patients, giving them instant access to their complete health records from anywhere in the state. According to The New York eHealth Collaborative (NYeC), "New York has been at the forefront of health innovation and will now be the first large start in the country to build a public network of interconnected electronic health records of this kind."

HealtheConnections will use the funding primarily to continue working with healthcare providers in the region to increase participation with the RHIO.


Population Health Improvement Program Award

The New York State Department of Health announced Friday, December 5 that HealtheConnections is the recipient of the Population Health Improvement Program (PHIP) award for the Central New York region. The award is for $2.4 million and will be used to help this region reach its goals of "better care, better population health, and lower health care costs."

Technical Talk: 2014 Highlights


Results Delivery
  • 62 new practices began receiving Results Delivery
  • A 45% increase was noted for results delivered, from 27,000 to over 60,000 results each month
  • Image-enabled Results Delivery (IERD) is now enabled to practices using Medent or athenahealth
CCDs
  • A new service in 2014, CCDs are now being submitted to the Community Repository, with 17 practices currently enabled and more being added
Direct (Secure) Messaging
  • As of November 2014, the total number of users increased from 223 in 2013 to 1,136. 
  • The total number of messages increased from 280 in 2013 to 21,540 as of November 2014
New Services
  • Full-cycle CCD exchange: allows the submission and retrieval of CCDs
  • Results Delivery via Direct (Secure) Messaging: patient results can be sent to providers through their secure messaging accounts
  • Alerts & Notifications: notifies providers of patient activity, such as ER visits

Madison County Launches Program to Prevent Diabetes 


The Madison County Rural Health Council is offering a national diabetes prevention program to help people with prediabetes learn more about healthy eating and physical activity to maintain a healthier lifestyle. 

This program is supported by Excellus BlueCross BlueShield and the New York State Health Foundation in collaboration with Herkimer County Healthnet.
