New HealtheConnections Board of Directors Selected

At the December 2nd Board of Directors meeting, the current HealtheConnections Board members voted to reduce the Board's size from 30 to 18 members. The 18 new members, who will officially be active in January 2016, are comprised of professionals across the healthcare field including hospital administrators, physicians, insurance/health plans, public and rural health representatives, but also local businesses and employers, consumers, and other independent members. Board members will be announced following the January 2016 meeting.

HealtheConnections President Rob Hack and current Board Chair Robert Weisenthal, MD would like to extend their gratitude and appreciation for the current board members, some of whom will serve on the new Board, who have fostered the growth of HealtheConnections and provided crucial guidance and support over the years. 

Refer a Colleague: Help us reach 600 participants by December 31


In July, HealtheConnections announced our 500th participating organization. Now, less than six months later, we are asking our current participants to refer a colleague to help us reach our 600th participating organization before we ring in the new year!

Let your colleagues know about the valuable services offered by HealtheConnections: 
  • Patient Lookup: Real-time consolidated view of a patient's community-wide health records
  • Direct Mail: Secure email system to communicate patient clinical information between healthcare professionals
  • myAlerts: Patient activity notifications sent to providers for Emergency Department encounters, in-patient hospital admissions and discharges
  • Results Access & Delivery: Automated delivery of results, reports, and images through the HIE to a participant's EHR, or viewed directly in the HIE via myResults
  • Image Exchange: Access to and viewing of diagnostic-quality radiology image study
  • Query-Based Exchange: Access to state and federal health records systems and other external partners to view patient data

New On HealtheCNY: Hospitalization Data


HealtheConnections is pleased to announce the addition of 42 new data indicators to!

These data indicators provide information at the regional, county, and zip code level about hospitalization and emergency department utilization in Cayuga, Cortland, Madison, Oneida, Onondaga, and Oswego counties.

Taken as a whole, these data provide important contextual information on our local healthcare delivery system. Preventable hospitalizations speak to the quality and accessibility of the preventive and primary care services available to the community.

Breastfeeding Topic Page Added to HealtheCNY

The new Breastfeeding Topic Page highlights an area of health promotion that is a New York State Prevention Agenda priority for several Central New York counties. Visit the Breastfeeding topic page on HealtheCNY to:
  • View data on breastfeeding rates across Central New York, including differences in breastfeeding rates across racial/ethnic groups
  • Compare your county's breastfeeding rates to other local counties
  • Learn about breastfeeding activities that are happening locally, and connect to these efforts
  • Find resources, best practices, and funding opportunities related to breastfeeding promotion

Spaces Still Available to Join Eligible Professional Expansion Program

The New York State Department of Health (NYS DOH), Office of Health Insurance Program (OHIP), with funding support from CMS, is offering an incentive program to healthcare professionals providing medical services to Medicaid patients. HealtheConnections, as a Regional Extension Center (REC) agent is administering this program. Participants are eligible for up to $3,500 of additional assistance, as well as Meaningful Use assistance and support.

This information supports the Medicaid Eligible Professional Expansion Program (EP2) and Meaningful Use activities and adoption of electronic health records (EHR). Related content is presented on behalf of NYeC for the promotion of the EP2 program and Meaningful Use.

CMS Extends Deadline for Submitting an Informal Review for 2014 Physician Quality Reporting System

If you have any questions regarding the status of your 2014 PQRS reporting or are concerned about potentially receiving the PQRS negative payment adjustment in 2016, please do not hesitate to submit an informal review request. CMS will be in contact with every individual eligible professional (EP) or PQRS group practice that submits a request for an informal review of their 2014 PQRS data. Additionally, revised 2014 Annual Quality and Resource Use Reports (QRURs) are now available via the CMS Enterprise Portal.

All informal review requests must be submitted electronically via the Quality Reporting Communication Support Page (CSP) which is available now through December 16, 2015 at 11:59 p.m. Eastern Time. 

Follow these steps to submit an informal review request:
  1. Go to the Quality Reporting Communication Support Page (CSP)
  2. In the upper left-hand corner of the page, under "Related Links," select "Communication Support Page"
  3. Select "Informal Review Request"
  4. Select "PQRS Informal Review"
  5. A new page will open
  6. Enter Billing/Primary Taxpayer Identification Number (TIN), Individual Rendering National Provider Identifier (NPI), OR Practice Site ID # and select "submit"
  7. Complete the mandatory fields in the online form, including the appropriate justification for the request to be deemed valid. Failure to complete the form in full will result in the inability to have the informal review request analyzed. CMS or the QualityNet Help Desk may contact the requestor for additional information if necessary.
For additional questions regarding the informal review process, contact the QualityNet Help Desk at 1-866-288-8912 (TTY 1-877-715-6222) or Monday-Friday from 7:00 a.m. to 7:00 p.m. Central Time. To avoid security violations, do not include personal identifying information, such as Social Security Number of TIN, in e-mail inquiries to the QualityNet Help Desk.

In the Community: Exhibits at Healthy Workplace Summit, Lights on the Lake

Healthy Workplace Summit & Awards
HealtheConnections attended the CNY Business Journal's Healthy Workplace Summit & Awards event on December 3 at the SRC Arena on the Onondaga Community College campus. Attendees spoke with HealtheConnections representatives about how to talk to their doctors about getting "HealtheConnected" as well as the importance of giving their consent to allow healthcare providers to view their electronic health records. 

Lights on the Lake
Throughout the holiday season, HealtheConnections will have a "greeting card" banner located along the Lights on the Lake trail through Onondaga Lake Park. Look for the holiday message at the beginning of the trail, and listen for the holiday greetings on Sunny 102! Lights on the Lake runs through January 3, 2016 every day from 5:00 - 9:00 pm.

Mike DiGiovanni Joins HealtheConnections as Manager of Provider Engagement

Mike DiGiovanni, Manager of Provider Engagement, has spent the majority of his professional career in the non-profit health sector. He has been responsible for corporate and volunteer development, special event management and sales for organizations such as the American Heart Association and the Cystic Fibrosis Foundation. DiGiovanni brings almost 20 years of professional cultivation and engagement experience to HealtheConnections. Welcome, Mike!

HealtheConnections is Hiring: Health Systems Improvement Analyst

HealtheConnections seeks a healthcare professional with direct clinical experience (RN, NP, PA, etc.) to serve on our Population Health Improvement team. This person will work to improve clinical outcomes through peer-to-peer academic detailing and practice facilitation with targeted healthcare providers and systems within a six county catchment area in Central New York (CNY) including Cayuga, Cortland, Herkimer, Oneida, Onondaga, and Oswego Counties. Interventions will focus on hypertension, type 2 diabetes, and/or people experiencing disproportionate risk for chronic disease because of racial/ethnic or socioeconomic disparities (inadequate access to care, poor quality of care, or low income).
