Connecting You to Better Care
January 2015 Issue

HealtheConnections Reaches 400 Participants

On December 22, 2014, Eye Care for the Adirondacks became HealtheConnections' 400th Participating Organization. Congratulations to the providers and staff!

In total, HealtheConnections finished the year with 408 Participating Organizations. This achievement would not have been possible without the support of our current participants, board members, and healthcare advocates across Central and Northern New York. Thank you for all you have done to make this a healthier region!

How Can HealtheConnections Help You?

HealtheConnections asked participants to share how their organization has benefited from using HealtheConnections services:

"HealtheConnections allows our agency to effectively and efficiently obtain information for Communicable Disease follow up. We did not have to call hospitals and providers to obtain information. A real time-saver!"

"Very helpful to get diagnoses and medical info on clients and check their recent hospital or doctor visits."

"Immediate electronic access to reports and results has greatly expedited the process of assembling patient records from other facilities. Electronic messaging other offices is much more convenient than calling."

"The ability to view records without having to wait for them to come to us is invaluable."

HIE Services: Year in Review and What's Coming Next

In 2014, HealtheConnections revealed new services and made enhancements to existing services:

Results Delivery
Providers in 62 new practices began receiving Results Delivery, while actual results delivered increased 45 percent - from 27,000 per month to over 60,000 per month. Users also saw a new enhancement, Image-Enabled Results Delivery (IERD), now enabled to practices using Medent and athenahealth. This service will be available for practices using other EMR vendors.

A new service in 2014 was the submission of CCDs to HealtheConnections' health information exchange (HIE). Twenty-four practices are enabled to-date, with more on the way in 2015.

Direct (Secure) Messaging
Direct users have increased over the past year from 223 to 1,136 along with the number of messages sent via direct messaging from 280 to 21,540.

As we transition into 2015, HealtheConnections looks forward to offering the following new services to our participants:

Alerts & Notifications: a customizable application that sends alerts triggered by specific events to the patient's provider. The initial pilot of the application will use inpatient admissions, emergency room registrations, and hospital discharges as events to trigger an alert. The alert can be configured to send a Direct message immediately, and can also be summarized in a daily report for viewing and delivery by Direct Mail. The provider will be able to select which event types and delivery method to use for their needs.

Full-cycle CCD Exchange: allows the submission and retrieval of CCDs.

Results Delivery via Direct (Secure) Messaging: the ability to receive results delivery through a direct message. This will be especially useful for practices that do not have an EMR.

Immunization Query: providers can pull full immunization reports directly from NYSIIS using HealtheConnections.

HIE Data Contribution Grows

HealtheConnections has been working with Medent to receive patient records into the HIE as a way to increase the available information to providers and referral partners across the community. There were 23 new data sources added in December, bringing the total number of Medent data sources to 41.
